As the parent or legal guardian of listed student(s), I hereby give permission for my child(ren) to participate in the International Grace Ministry (“IGM”) Hallelujah Night. I understand that IGM is a nonprofit organization, which is presenting this program for my child, other participants, and the community. I also understand that the program has activities that can involve physical contact with other participants, the ground or equipment, walls or fixtures, and other objects or animals, which may result in physical injury to my child. I have explained the risks and benefits of participating in this program to my child and my child is in proper physical condition and has no existing injuries or conditions that could jeopardize his/her safety or health, or the safety or health of the other participants.
In consideration for allowing my child to participate in this VBS, on behalf of myself, my spouse, my child(ren), and any other person with standing to sue, I fully release and discharge all liability for any harm or injury suffered directly or indirectly as a result of my child’s participation in IGM’s VBS and promise not to sue IGM, its representatives, staff, volunteers, employees, officers, directors, or any other agent (collectively “IGM’s Staff”) from any and all claims, including but not limited to, claims of negligence.
I also give permission to IGM’s Staff to administer first aid or to seek medical care for my child during my child’s participation in the program, including transportation of my child to a medical facility for additional treatment. If my child needs medical treatment, I agree to be financially responsible for any costs incurred as a result of such treatment.
I understand that this document is written to be as broad and inclusive as legally permitted by the law. I agree that if any portion is held invalid or unenforceable, I will continue to be bound by the remaining terms. I fully understand the legal consequences of signing this document, including (a) releasing IGM and IGM’s Staff from all liability, (b) promising not to sue IGM or IGM’s Staff, and (c) assuming all risks of participating in this VBS Program.
I have read and understood this document, and I am signing it freely. By signing this document, I further represent that I am authorized to sign this document on behalf of the above referenced child(ren).
